Rezumat
Factorii de ordin infecţios, traumatic, toxic sau degenerativ din alte organe induc
fie leziuni patognomonice în structurile sistemului nervos central (exemplu: rabia,
scrapia), fie modificări ale organizării cito-histologice complexe a ţesutului nervos şi
determină apariţia unor procese patologice caracterizate, în special, prin modificări la
nivelul neuronilor şi la nivelul elementelor celulare gliale ca leziuni asociate, cu
semnificaţie diagnostică în coroborare cu datele epizootologice şi alte investigaţii de
laborator.
Prin examen histopatologic se urmăreşte punerea în evidenţă a modificărilor
structurale esenţiale sub aspect morfologic şi histologic, în vederea identificării
mecanismelor generatoare de tulburări la nivel local şi impactul acestora asupra
întregului organism, corelând manifestările clinice ale animalului bolnav (când acestea au
fost surprinse) cu leziunile decelate.
Modificările structurale evidenţiate la nivel macroscopic şi microscopic stau la
baza interpretării leziunilor produse de agenţii patogeni sau vulneranţi, ierarhizând
contribuţia lor la dezvoltarea leziunilor în dinamică sau prin efect cumulativ când acesta
există.
Formele nervoase sunt semnalate frecvent la ovine, dar în acelaşi timp sunt foarte
uniforme ca manifestare clinică şi morfopatologică. Pentru acest motiv, cercetarea
modificărilor histopatologice ale sistemului nervos central în vederea stabilirii frecvenţei,
localizărilor preferenţiale, tipului lezional predominant sau descoperirii unor leziuni
specific etiologice, au importanţă practică în stabilirea diagnosticului diferenţial.
Encefalopatiile spongiforme transmisibile, alături de altă zoonoză majoră cu
localizare nervoasă la ovine, rabia, sunt bolile pentru care sunt prevăzute investigaţii de
laborator obligatorii ale sistemului nervos central la ovine în cadrul Programul acţiunilor
de supraveghere, prevenire, control şi eradicare a bolilor la animale, a celor
transmisibile de la animale la om, elaborat de Autoritatea Naţională Sanitară Veterinară
Bucureşti.

Din 1990 până în prezent, Comisia Europeană a adoptat o serie de măsuri de
protecţie a sănătăţii umane şi animale de riscul de contaminare cu encefalopatii
spongiforme transmisibile (EST). Aceste măsuri au avut la bază dispoziţiile de
salvgardare ale directivelor privind măsurile de control veterinar.
Având în vedere magnitudinea riscului pe care îl reprezintă anumite forme de
EST, Comisia prevede programe de monitorizare sporită pentru caprine şi ovine, în urma
detectării cazurilor de encefalopatie spongiformă bovină (ESB) la o capră în 2005 şi a trei
cazuri neobişnuite de EST la oi, cazuri pentru care nu putea fi exclusă ESB.
Structura cercetărilor. Teza cuprinde un număr de 248 pagini şi este structurată în
conformitate cu prevederile legale actuale; în conţinutul acesteia sunt inserate un număr
de 121 figuri (imagini, grafice, diagrame) şi 23 de tabele. În partea I sunt prezentate cele
mai importante şi concludente date din literatura de specialitate referitoare la aspecte
histologice, fiziologice, citopatologice ale țesutului nervos şi aspecte referitoare la
encefalopatiile spongiforme transmisibile la rumegătoarele mici – etiologia prionică,
patogeneză, diagnostic clinic şi de laborator. În partea a II-a sunt prezentate cercetările
proprii ce au vizat studii de laborator pentru monitorizarea scrapiei (capitolul II.1),
studiul histopatologic (capitolul II.2) al sistemului nervos central efectuat pe un număr
determinat de probe de creier, în vederea identificării de modificări/leziuni ale ţesutului
nervos, altele decât cele specifice encefalopatiilor spongiforme transmisibile şi, în final,
confirmarea a două focare de scrapie (capitolul II.3). Partea bibliografică cuprinde un
număr de 201 titluri de lucrări ştiinţifice din literatura de specialitate autohtonă şi străină.
Studii de laborator pentru monitorizarea scrapiei
În capitolul II.1. sunt evaluate rezultatele investigaţiilor de laborator în direcţia
scrapiei şi sunt comparate metodele de diagnostic între ele sub aspectul sensibilităţii şi
specificităţii, respectiv decelarea şi cuantificarea acumulărilor de proteină prionică
anormală PrPres rezistentă la proteinaza K în ţesutul nervos, prin teste imunoenzimatice şi
evidenţierea prezenţei/absenţei leziunilor provocate de convertirea PrP în PrPres şi
acumularea acesteia, prin examene histopatologice.

Studiul a avut în vedere testele de laborator la care au fost supuse probele de creier
de ovine pentru supravegherea şi/sau monitorizarea formelor de encefalopatii
spongiforme transmisibile la rumegătoarele mici, în judetul Cluj între anii 2004-2010.
Pe parcursul anilor luaţi în studiu a fost examinat un număr total de 1540 ovine, cu
respectarea grupelor „ţintă” stabilite de prevederile legislaţiei referitoare la EST.
Astfel, un număr de 610 probe au provenit de la ovine moarte, 594 probe de la
ovine sacrificate de urgenţă (în această categorie înscriindu-se şi ovinele sacrificate în
scop de confirmare de diagnostic sau ca urmare a necesităţii eliminării din efectiv, cum
este cazul celor cu diagnostic de epididimită infecţioasa, maedi-visna), iar un număr de
336 probe au provenit de la ovine clinic sănătoase sacrificate pentru consum uman
(sacrificare tradiţională pentru consum familial).
Urmărind precizările tehnice ale Programului pentru eradicarea şi monitorizarea
encefalopatiilor spongiforme transmisibile, în cazul probelor de creier recoltate de la
animalele moarte şi de la animalele sacrificate de urgenţă (cu excepţia celor improprii
procesării histologice) s-a efectuat atât screening-ul rapid, cât şi examenul histologic,
conform procedurii specifice.
Screening-ul rapid s-a făcut utilizând metoda BIO-RAD TeSeE, metodă validată în
laboratorul de histopatologie al LSVSA Cluj şi acreditată de către Organismul Naţional de
Acreditare din România (RENAR.). Este o metodă imunoenzimatică ce utilizează 2 anticorpi
monoclonali şi permite determinarea calitativă a proteinei anormale prionice PrPres
rezistentă la proteinaza K, în ţesutul nervos central, după denaturarea şi concentrarea
proteinelor.
Histopatologic, pentru fiecare probă s-au examinat în total câte 20 de secţiuni din
bulb rahidian, protuberanţă anullară, mezencefal cu zona de colliculus anterior, cerebel,
cortex cerebral şi măduva rahidiană din zona cervicală, procesate prin tehnica la parafină
şi colorate Hematoxilină Eozină.
Astfel, din totalul de 1540 probe de creier supuse examinarii prin teste rapide, un
număr total de 541 au fost supuse şi examenului histopatologic pentru evidenţierea
leziunilor caracteristice scrapiei, după cum urmează: 330 probe de creier prelevate de la
cadavre (ovine moarte în ferme, gospodăriile populaţiei, pe păşune, în timpul
transportului, în abator, sau în cazul unor accidente, indiferent de natura acestora), 208

probe creier prelevate de la ovine sacrificate de urgenţă (ovine la care examenul clinic
ante-mortem a stabilit prezenţa unor manifestări nervoase sau faptul că nu se pot deplasa,
stau culcate, incapabile să se ridice sau să meargă fără asistenţă; ovine identificate de
medicul oficial în abator la inspecţia ante-mortem cu semne clinice de boală, indiferent
de natura bolii: boli infecţioase, medicale etc ) şi un număr de 3 probe creier recoltate de
la ovine sacrificate pentru consum uman (familial).
Din totalul de 1540 teste imunoenzimatice, 1527 teste au prezentat densităţile
optice mai mici decât valoarea prag (cut-off-ul), înregistrând deci rezultate negative la
prima testare. Un număr de 13 teste au prezentat iniţial densităţi optice situate chiar sub
valoarea prag şi au fost retestate în duplicat pornind de la omogenatul iniţial. Pentru toate
cele 13 cazuri rezultatele au fost negative pentru ambele duplicate.
În cazul celor 541 probe de creier procesate histopatologic prin tehnica la parafină
nu s-au evidenţiat leziuni specifice encefalopatiilor spongiforme transmisibile, în niciunul
din fragmentele de ţesut nervos examinate.
În urma acestor examene (screening rapid, examene histopatologice) s-a stabilit că
probele de creier analizate în perioada 2004-2010 proveneau de la ovine libere de
encefalopatii spongiforme transmisibile (scrapie clasică, scrapie atipică).
Studiu histopatologic
În capitolul II.2. sunt prezentate rezultatele unui studiu histopatologic al sistemului
nervos central pe un număr determinat de probe de creier (100) prelucrate histologic
pentru monitorizarea scrapiei, urmărind analiza incidenţei modificărilor / leziunilor
(altele decât cele specifice EST) în sistemul nervos central la categoriile “ţintă” de ovine
examinate, clasificarea acestora în funcţie de caracteristicile histologice predominante,
realizarea unei ierarhizări comparative, evaluarea repartiţiei şi frecvenţei leziunilor
microscopice în diferitele segmente de ţesut nervos examinate, stabilirea unei corelaţii
între modificările/leziunile identificate şi starea de sănătate sau/şi existenţa unor leziuni la
distanţă.
Studiul a identificat modificări/leziuni de diferite tipuri ale ţesutului nervos la 73%
din cazurile examinate, grupate în funcţie de predominanţa aspectelor histopatologice
degenerative, vasculare, reacţii celulare neinflamatorii/inflamatorii.

Sintetic, rezultatele examinării microscopice au identificat modificări degenerative
ale corpului celulei nervoase (modificări ale nucleului, citoplasmei), tulburări vasculare
(edem, congestii, hemoragii, proliferări ale celulelor endoteliale, tromboze vasculare),
reacţii celulare (infiltraţii cu mononucleare, infiltraţii cu granulocite, proliferări ale
celulelor gliale, ale macrofagelor, prezenţa celulelor gigante), prezenţa incluziilor virale,
imagini de moarte celulară (tigroliză, satelitoză, neuronofagie), tulburări metabolice cu
acumulare de pigment melanic, prezenţa granuloamelor de tip parazitar.
Realizând o ierarhizare descrescândă a tipurilor de modificări/leziuni din diferitele
segmente de sistem nervos central pentru cele 100 cazuri analizate au fost constatate
următoarele:
Cele mai numeroase leziuni au fost cele de tip vascular, rezultatul examinării
microscopice fiind un total de 177, din care imaginile de edem au fost întâlnite de 84 de
ori, congestiile de 53 de ori, hemoragiile de 8 ori; au fost surprinse 29 de imagini cu
proliferări ale celulelor endoteliului vascular şi 3 imagini de tromboză vasculară.
În ierarhie urmează modificările degenerative ale pericarionilor cu un număr total
de 93 de imagini, de tip “cromatolitic“(45 imagini care frecvent au putut fi interpretate
ca indicator al unor degenerescenţe nespecifice combinate cu pierderea afinităţii
tinctoriale), degenerescenţe granulare şi vacuolare (31) şi atrofie neuronală cu pierderea
volumului citoplasmatic şi reducerea în volum (17).
Reacţiile celulare au fost surprinse în număr total de 84 şi au constat în infitraţii cu
mononucleare (37), infiltraţii cu granulocite neutrofile (16), gliocitoză (14), inifiltraţii cu
eozinofile (13), proliferări ale macrofagelor (2), prezenţa celulelor gigante (2).
Incluziile virale au fost surprinse de 2 ori, situate intracitoplasmatic.
Au fost surprinse 28 de imagini de moarte celulară cu satelitoză (11), neuronofagie
(9), tigroliză (8).
Acumularea de pigment melanic a fost surprinsă de 3 ori la nivelul
leptomeningelui.
Analizând repartizarea tipurilor de modificări/leziuni pe categorii de ovine, la
ovinele moarte şi la cele sacrificate de urgenţă care reprezintă 97% din materialul de
studiu, au rezultat următoarele:

La ambele categorii de ovine au predominat leziunile de tip vascular, urmate de
modificările degenerative ale pericarionilor.
În cazul probelor de creier provenite de la ovinele sacrificate de urgenţă s-au
înregistrat reacţii celulare pentru toate tipurile celulare (mononucleare, granulocite
neutrofile, celule gliale, eozinofile). Prezenţa celulelor macrofage şi a celulelor gigante
de tip parazitar a fost surprinsă în aceeaşi măsură pentru ambele categorii.
Imaginile de moarte celulară au fost mai frecvent întâlnite la sacrificările de
urgenţă, comparativ cu preparatele histologice provenite de la cadavre.
Acumularea de pigment melanic a fost întâlnită la preparatele histologice de la
cadavre, dar nu a fost întâlnită la preparatele obţinute de la ovinele sacrificate de urgenţă.
Granuloamele parazitare au fost întâlnite în egală măsură la cele două categorii de
ovine.
În cazul examinării preparatelor histologice provenite de la ovinele sacrificate
pentru consum uman, s-au întâlnit: imagini de tip cromatolitic (2), congestii (2), infiltraţii
cu mononucleare (2), edem (1), satelitoză (1), melanoză (1).
Evaluarea statistică privind incidenţa localizării principalelor tipuri de leziuni
microscopice în segmentele de sistem nervos central examinate a relevat următoarele:
Modificările degenerative ale pericarionului au afectat în primul rând bulbul
rahidian (22,31%), apoi, aproximativ în egală măsură, cortexul cerebral (19,83%),
cerebelul (19%), puntea (18,18%) şi tuberculii cvadrigemeni (16,53%). Într-o măsură
mult mai redusă au fost afectaţi neuronii din hipocamp sau măduva rahidiană cervicală.
Se constată o incidenţă mai crescută a leziunilor neinflamatorii, constând în edem,
congestii, hemoragii, cu precădere în zona tuberculilor cvadrigemeni (29,37%), bulbului
(22,03%), cerebelului (19,21%), cortexului cerebral (12,99%,) şi punţii (11,30%), în timp
ce preponderenţa acestora este mult mai mică în măduva rahidiană cervicală (3,38%) şi
hipocamp (1,69%).
În cazul leziunilor inflamatorii bulbul rahidian a fost cel mai afectat, în proporţie
de 34,14%, apoi tuberculii cvadrigemeni, în proporţie de 24,39%, cortexul cerebelos şi
cortexul cerebral în proporţie de 19,31%, respectiv, 12,50%, În procent mult mai redus au
fost sediul leziunilor inflamatorii, hipocampul cu 4,55%, măduva rahidiană şi puntea cu
câte 3,41% şi 2,27%.

Depunerea de pigment melanic a fost identificată în 100% din cazuri cu localizare
în leptomeningele cerebelului şi al emisferelor cerebrale.
Nu există diferenţe semnificative statistic sub raportul frecvenţei cu care se
localizează modificările/leziunile în segmentele de ţesut nervos examinate. Bulbul
rahidian şi tuberculii cvadrigemeni sunt segmentele afectate în cea mai mare măsură, în
timp ce hipocampul şi măduva rahidiană sunt afectate în cea mai mică măsură. Cortexul
cerebral şi cerebelul sunt sediul modificărilor aproximativ cu aceeaşi frecvenţă. În punte
se remarcă prezenţa modificărilor degenerative ale pericarionilor şi a leziunilor
neinflamatorii şi într-o măsură mult mai mică se produc leziuni inflamatorii.
Leziunile de tip vascular au predominat la 41% din cazuri, urmate în ierarhie de
leziunile inflamatorii, cu un procent de 16%. În această categorie s-au încadrat
inflamaţiile de natură parazitară (8%), inflamaţiile de natură bacteriană (6%) şi
inflamaţiile de natură virală (2%). La 11% din cazuri au fost identificate modificări a
căror etiologie nu a putut fi precizată şi care au fost atribuite unor afecţiuni şi stări
patologice la nivelul unor organe viscerale, de natură metabolică, infecţioasă şi/sau
parazitară. A fost observată acumularea de pigment melanic în 3% din cazuri şi
modificări ale lichidului cefalo-rahidian în 2% din cazuri. Nu s-au observat modificări ale
ţesutului nervos în segmentele de sistem nervos central examinate în 27% din cazuri.
Edemul neinflamator a fost diagnosticat ca modificare predominantă la 37 de
cazuri şi a constat în imagini de lărgire a spaţiului Wircoff-Robin, uneori cu prezenţa
unui lichid slab colorat, cu aspect vacuolar; capilarele sau arteriolele aveau lumenul redus
datorită transsudatului. Baza edemului vasogenic o reprezintă lezionarea endoteliului
vascular, cu o severitate care permite creşterea permeabilităţii vasculare. Fluidul se
dispersează între celule, ca răspuns la presiunea hidrostatică de la nivelul circulaţiei
cerebrale şi a celei de la nivelul altor ţesuturi
S-au constatat diferenţe de susceptibilitate la edem, între materia cenuşie şi cea
albă, iar vulnerabilitatea diferitelor zone cu materie cenuşie sau albă a fost, de asemenea,
diferită şi a constat în imagini de edem intracelular tradus prin hipertrofia astrocitelor,
acompaniată uneori de reacţia oligodendrocitelor de la nivelul materiei albe (nucleu
mărit, mai puţin dens, hipocrom); edem vasogenic (intercelular) fără modificări majore la

nivelul substanţei cenuşii, dar materia albă, care oferă o rezistenţă mai scăzută la trecerea
fluidului de edem, apare texturată, cu aspect vacuolar.
Frecvent, s-a constatat edem sub epiteliul canalului ependimar, celulele epiteliale
fiind invadate de lichid, nucleul celulelor împins spre polul apical, uneori chiar picnotic;
alteori, celulele epiteliale erau desprinse, iar substanţa nervoasă limitrofă avea aspect
spongios.
Cerebelul, mai cu seamă în zona celulelor Purkinje, a fost sediul unui edem ce
disloca şi compresa celulele. Adesea edemul a fost acompaniat de o discretă gliocitoză
şi/sau limfocitoză.
Hemoragiile au predominat la 4 cazuri, fiind întâlnite aspecte de hemoragii prin
diapedeză, fără specificitate în distribuţia topografică, cu eritrocitele rămase în spaţiul
perivascular, însoţite sau nu de hipertrofia celulelor endoteliale şi a nucleilor, dar şi
hemoragii per rexis cu localizare bilaterală şi simetrică în cortexul cerebral. La două
dintre aceste cazuri examenul bacteriologic a fost pozitiv pentru Clostridium spp., iar un
caz prezenta infestaţie masivă cu larve de Oestrus ovis ajunse eratic în circulaţia
craniană. Imaginile de edem şi hemoragii nu au fost însoţite de modificări de tip
inflamator.
S-a acordat atenţie deosebită leziunilor inflamatorii ale sistemului nervos central,
foarte importante pentru patologia veterinară deoarece majoritatea lor sunt fatale şi pentru
că examenul histologic trebuie să orienteze sau să stabilească diagnosticul etiologic.
Inflamaţiile parazitare au reprezentat jumătate (50%) din leziunile inflamatorii
identificate. Inflamaţiile bacteriene au fost diagnosticate ca şi inflamaţii listerelice în
proporţie de 25% şi inflamaţii purulente de alt tip 12,5%. Inflamaţiile virale au fost
identificate în proporţie de 12,5% şi au avut etiologie rabică.
Diagnosticarea histopatologică a encefalitelor parazitare s-a corelat, în cazul a 6
ovine sacrificate de urgenţă, cu semnalarea tulburărilor nervoase de focar cu învârtiri în
manej şi tulburări de echilibru ca urmare a compresiunii exercitate în emisferele
cerebrale, respectiv în cerebel sau în bulbul rahidian.
Tabloul morfopatologic a fost în raport cu stadiul evolutiv al bolii, prezentând un
polimorfism accentuat. Am identificat prezenţa infiltratului de tip neutrofilic şi
eozinofilic; în plus, la două dintre cazuri, granuloame parazitare cu celule gigante, reacţie

vasculo-conjunctivă şi macrofage cu citoplasmă ‘spumoasă‘, fiind surprinse şi resturi de
paraziţi; 3 dintre cazuri au fost diagnosticate ca encefalite parazitare de migrare, cu
focare neregulate de necroză, infiltraţie eozinofilică; la 2 cazuri encefalita parazitară a
fost acompaniată de leptomeningită eozinofilică. Leziunile produse de paraziţi au fost
întâlnite în bulb, tuberculii cvadrigemeni, cortexul cerebral, punte, hipocamp şi măduva
rahidiană cervicală.
La 2 ovine moarte, documentele sanitar veterinare insoţitoare nu au precizat
simptomatologie nervoasă, probele de creier fiind procesate doar pentru monitorizarea
scrapiei, deşi studiul histopatologic a evidenţiat leziuni în stadii avansate, soldate cu
dezvoltarea celulelor gigante, multinucleate, cu citoplasma axată pe masa de detritus şi
încărcată de vacuole şi resturi tisulare, precum şi prezenţa de trombi parazitari.
Encefalitele de natură bacteriană au reprezentat 37,5% din leziunile inflamatorii,
din care, 25% au fost identificate ca fiind de natură listerelică.
La o ovină fără semne clinice, sacrificată pentru consum uman inflamaţia a fost
predominant monocitară, ceea ce denotă un stadiu acut al infecţiei care, în mod logic, ar
fi trebuit să fie însoţit de simptomatologie nervoasă. Bulbul rahidian prezenta
perivascularită monocitară sub formă de manşoane groase, iar în cerebel s-a identificat
focar masiv şi infiltraţie difuză, intensă de limfocite, edem în neuropil şi perivascular,
neuronoliză, perivascularită monocitară.
La ovinele sacrificate de urgenţă deoarece prezentau clinic poziţii anormale ale
capului, leziunile au fost localizate la nivelul bulbului rahidian şi cerebelului şi au constat
în focare de necroză de lichefiere, cu prezenţa constantă a infiltratului neutrofilic,
monocitar şi a manşoanelor perivasculare limfohistiocitare.
Un procent de 12,5% din inflamaţiile de natură bacteriană au fost observate la
ovine sacrificate de urgenţă (2 cazuri) şi au fost encefalite purulente, cu infiltraţie masivă
cu granulocite neutrofile, leptomeningită şi prezenţa de bacterii dispuse în cuiburi, cu
localizare în bulbul rahidian la un caz şi în cerebel la cel de-al doilea caz; în ambele
cazuri examenul bacteriologic a fost pozitiv pentru tulpini β-hemolitice de E. Coli.
Stadiul infiltrativ cu caracter de exudat purulent s-a corelat cu durata bolii, ambele cazuri
diagnosticate clinic manifestându-se prin simptomatologie nervoasă şi decubit prelungit.

Diagnosticul de encefalită virală respectiv, rabie a fost stabilit la 2 cazuri care au
prezentat simptomatologie nervoasă şi care au fost sacrificate de urgenţă. În ambele
cazuri, pe lângă encefalita limfohistiocitară, au fost identificaţi corpusculii Babeş Negri.
Leziunile alterative şi de proliferare celulară au fost de intensitate moderată comparativ
cu manifestările clinice.
Encefalitele nespecifice au fost descrise la 5 ovine sacrificate de urgenţă şi la 6
ovine moarte, cu intensitate puţin relevantă şi au fost considerate ca fiind asociate unor
stări de boală cu localizări în alte organe. Au fost identificate modificări degenerative ale
pericarionilor (imagini de nuclei poziţionaţi excentric, cromatoliză periferică şi/sau
central cu evidenţierea granulelor Nissl, picnoză, rexă şi liză neuronală cu modificări la
nivelul astrocitelor şi endoteliilor capilare din vecinătatea neuronilor afectaţi) din bulbul
rahidian, tuberculii cvadrigemeni, cortexul cerebral şi cerebelos, proliferări gliale,
neuronofagie cu fagocite microgliale adunate în jurul fragmentelor degenerate ale
corpilor neuronali, edem perivascular şi/sau în substanţa nervoasă, hemoragii prin
diapedeză, prezenţa infiltratului eozinofilic în lumenul capilarelor şi arteriolelor.
Melanoza a fost surprinsă la 2 cadavre şi la o ovină sacrificată pentru consum
uman. Depunerea de pigment melanic s-a manifestat într-o manieră analogă cu alte boli
de depozitare lizozomală, o membrană limitatoare bine definită nefiind evidentă şi a fost
identificată in leptomeningele cerebelului şi al emisferelor cerebrale; la un caz
melanocitele au fost prezente si perivascular; la niciunul dintre cazuri nu s-au identificat
melanocite în substanţa nervoasă.
La 2 cadavre am constatat o creştere a lichidului cefalorahidian, tradus prin
dilatarea aqveductului, uneori asociat cu edemul substanţei nervoase limitrofe.
În sinteză, cele mai afectate de modificările/leziunile ţesutului nervos au fost
ovinele moarte, respectiv 40 de cazuri, apoi ovinele sacrificate de urgenţă, 31 de cazuri,
iar dintre ovinele sacrificate pentru consum uman, deşi au fost identificate 2 cazuri cu
leziuni, trebuie subliniat numărul extrem de mic de examene histopatologice efectuate la
această categorie de ovine şi anume, 3. Mai mult, cele trei cazuri au fost procesate
histologic în scopul obţinerii unor preparate de ţesut nervos de referinţă pentru
examinarea microscopică, ele fiind recepţionate în laboratorul de morfopatologie cu

documente însoţitoare care atestau starea de clinic sănătos, sacrificate pentru consum
uman, iar capetele au fost dirijate pentru screening rapid prin test imunoenzimatic.
Modificări/leziuni în ţesutul nervos au fost diagnosticate la ovine sacrificate de
urgenţă care nu au manifestat simptomatologie nervoasă sau modificări macroscopice ale
sistemului nervos central şi la ovine clinic sănătoase sacrificate pentru consum.
Studiul histopatologic a scos în evidenţă o incidenţă mai ridicată a
modificărilor/leziunilor sistemului nervos central faţă de cazuistica clinică; acestea sunt
frecvent întâlnite la ovine, de multe ori evoluând asimptomatic sau semnele clinice sunt
puţin evidente, ceea ce duce la nesemnalarea lor de către proprietar sau chiar de către
medicii veterinari.
Prezenţa modificărilor/leziunilor asociate din sistemul nervos central cu leziuni
primare din alte ţesuturi şi organe arată că bariera hematoencefalică există, dar nu pentru
orice stare patogenă din organism.
Confirmarea a două focare de scrapie
Din totalul de 158 probe creier examinate în anul 2011 prin testul imunoenzimatic
BIO-RAD, un procent de 39,24%, adică un număr de 62 de probe au fost procesate şi
histologic pentru evidenţierea leziunilor specifice encefalopatiilor spongiforme
transmisibile (scrapie clasică, scrapie atipică).
Diagnosticarea primelor cazuri pozitive de scrapie în Laboratorul de
Morfopatologie specializat pentru Diagnosticul Encefalopatiilor Spongiforme
transmisibile din LSVSA Cluj, în anul 2011, a fost motivaţia continuării cercetărilor
privind formele de encefalopatii spongiforme transmisibile la rumegătoarele mici.
În capitolul II.3 sunt prezentate rezultatele evaluării protocolului de diagnostic
adoptat în cele 2 cazuri pozitive, reprezentante a 2 categorii “ţintă”, rezultatele comparării
performanţelor testelor de laborator efectuate la aceste cazuri pozitive, precum şi aspecte
epidemiologice ale scrapiei în judeţul Cluj, în contextul evoluţiei acestei boli pe teritoriul
României.
Categoriile “ţintă” reprezentate de cele 2 cazuri pozitive au fost ovinele clinic
sănătoase sacrificate pentru consum uman şi ovinele sacrificate de urgenţă în urma
examenului clinic la care s-a constatat simptomatologie nervoasă cu ataxie, anteropulsie

cu sprijinul capului de diferite obiecte, tremor generalizat, torticolis, prurit accentuat
automutilant la nivelul membrelor superioare, ataxie, bruxism, slăbire progresivă,
decubit.
Ca urmare a încadrării în cele 2 categorii “ţintă”, conform protocolului obligatoriu
de lucru, s-a procedat la prelevarea diferită de probe şi direcţionarea pentru investigaţii de
laborator.
De la ovina cu simptomatologie nervoasă a fost prelevat creierul în întregime
după decalotare, iar protocolul de lucru a constat în prelevarea eşantionului de ţesut
nervos din regiunea obex pentru screening rapid scrapie prin test imunoenzimatic;
fragmente punte, cortex cerebral, cerebel pentru examen histopatologic scrapie,
fragmente trunchi cerebral (ţesut nefixat, conservat prin congelare) pentru testul de
confirmare Western Blot şi fragmente din punte pentru imunohistochimie scrapie.
De la ovina clinic sănătoasă a fost recoltat trunchiul cerebral prin tehnica foramen
magnum şi s-au prelevat: eşantion de ţesut nervos din regiunea obex pentru test
imunoenzimatic scrapie şi fragmente trunchi cerebral pentru testul de confirmare Western
Blot.
Testul de confirmare Western Blot s-a realizat prin digestia proteinei prionice
celulare (PrPc), urmată de purificarea şi concentrarea PrPres specifică de boală. Detecţia
PrPres s-a efectuat prin migrare electroforetică urmată de imunoblotare.
Testul de imunohistochimie scrapie s-a realizat pe secţiunile histologice
permanente folosind un anticorp monoclonal cu specificitate înalta pentru PrPres -
anticorp primar monoclonal pe şoarece antiPrPSC.
Testele imunoenzimatice au avut ca rezultat la testarea iniţială, pentru ambele
probe, valori ale densităţilor optice peste valoarea prag (cut-off-ul), rezultând rapoarte
mari în favoarea densităţilor optice înregistrate de probele în cauză, comparativ cu cut-
off-ul (OD/C.OFF- 1.768/0.218 cazul cu simptomatologie nervoasă; 1.692/0.220 cazul
fără semne clinice).
Pentru verificarea acurateţii rezultatelor obţinute, pornind de la omogenatul iniţial,
s-au retestat ambele probe prin test imunoenzimatic, în duplicat. La cazul cu
simptomatologie nervoasă valoarea densităţilor optice a fost cu mult peste valoarea prag,
rezultând rapoarte over în favoarea densităţilor optice înregistrate ale probei, comparativ

cu cut-off-ul (3.601, 3.634/0.219). S-au obţinut densităţi optice peste valoarea prag (cut-
off-ul) şi la cazul fără semne clinice (1.670, 2.306/0.220). Rezultatele obţinute, au fost
interpretate ca pozitive pentru scrapie.
Valoarea foarte mare a densităţilor optice obţinute la eşantioanele de ţesut nervos
prelevate de la cazul cu simptomatologie nervoasă se datorează cantităţii mult mai
crescute de PrPres detectată în ţesutul nervos, comparativ cu cantitatea de PrPres prezentă
în ţesutul nervos la cazul fără semne clinice.
Obţinerea densităţilor optice cu valoare mare, peste valoarea prag, la ovina clinic
sănătoasă, sacrificată pentru consum uman, demonstrează sensibilitatea şi specificitatea
testului imunoenzimatic de screening rapid în detecţia proteinei prionice anormale PrPres
în ţesutul nervos infectat.
Pentru ambele probe, imunoblotarea a dus la obţinerea profilelor specifice cu
prezenţa unei pete mari doar pe banda corespunzătoare proteinelor cu greutate moleculară
de 27-30 kDa, prezente în proba tratată cu proteinază K. Profilele rezultate prin migrare
electroforetică, urmată de imunoblotare, demonstrează digestia PrPc şi transformarea
proteinei prionice specifice de boală într-un fragment rezistent la proteinază, cu masă
moleculară redusă prin îndepartarea capătului amino-terminal al proteinei; cele 2 benzi
superioare corespund formelor mono şi di-glicozilate (27-30 kDa), iar banda inferioară
corespunde formei neglicozilate.
Examinarea histopatologică a identificat prezenţa de vacuolizări specifice
localizate în pericarionul neuronilor şi în neuropilul substanţei cenuşii, cu distribuţie
simetrică, bilaterală. Pericarionii s-au prezentat destinşi de una sau mai multe vacuole,
rotunde sau ovale, cu contur net şi regulat care fuzionează, rezultând aspectul spongiform
al materiei cenuşii. S-au întâlnit şi imagini de necroză neuronală, neuroni cu nuclei
picnotici, citoplasmă omogenă, intens eozinofilă, discretă proliferare glială, precum şi
depuneri de amiloid perivascular; nu s-au observat reacţii inflamatorii. Nucleul dorsal al
vagului s-a dovedit a fi sediu de elecţie pentru vacuolizările specifice.
Evidenţierea leziunilor demonstrează că pătrunderea prionilor în sistemul nervos
central este urmată de convertirea proteinei normale PrPc, produsă de neuroni şi celule
gliale, în PrPsc şi acumularea acesteia, urmată apoi de modificările histopatologice
caracteristice, responsabile de tulburările clinice.

Imunohistochimia efectuată la cazul cu simptomatologie nervoasă, în secţiunile
histologice realizate din fragmentele de trunchi cerebral a identificat depozite
punctiforme, în granule de PrPres, dispuse arbitrar, în mai multe focare sau difuze în
neuropil şi pericarioni, cu o intensitate şi distribuţie a imunoreactivităţii diferite.
Analiza rezultatelor demonstrează sensibilitatea şi specificitatea testului
imunoenzimatic de screening rapid în detecţia proteinei prionice anormale PrPres în
ţesutul nervos infectat, prin detectarea bolii la ovina clinic sănătoasă, sacrificată pentru
consum uman şi că există corelaţie între cantitatea de proteină prionică anormală PrPres
acumulată în ţesutul nervos şi declanşarea semnelor clinice, fapt evidenţiat de valoarea
foarte mare a densităţilor optice obţinute la eşantioanele de ţesut nervos prelevate de la
cazul cu simptomatologie nervoasă, comparativ cu cele obţinute la cazul fără semne
clinice.
Studiul a scos în evidenţă că scrapia poate evolua şi în absenţa semnelor clinice
ceea ce înseamnă că infecţiile prionice oculte la animale sunt mult mai răspândite decât
cazurile clinice cunoscute. Având în vedere că diagnosticarea cazului pozitiv la ovinele
sacrificate pentru consum uman s-a făcut în cadrul monitorizării de rutină, subliniază
importanţa acesteia.
Concluzii generale
1. Studiul histopatologic, soldat cu depistarea de modificări/leziuni ale sistemului
nervos central la 73% din cazurile examinate, scoate în evidenţă importanţa
examinării acestui ţesut cu mai mare atenţie, atât macroscopic, cât şi prin
prelevare de probe pentru studiu histopatologic în vederea identificării leziunilor
primare, dar şi a celor asociate.
2. Modificări/leziuni în ţesutul nervos au fost diagnosticate la ovine sacrificate de
urgenţă care nu au manifestat simptomatologie nervoasă sau modificări
macroscopice ale sistemului nervos central şi la ovine clinic sănătoase sacrificate
pentru consum uman.
3. Factorii de ordin infecţios, traumatic, toxic sau degenerativ din alte organe induc
modificări ale organizării cito-histologice complexe a ţesutului nervos cu apariţia

unor procese patologice, demonstrând că bariera hematoencefalică există, dar nu
pentru orice stare patogenă din organism.
4. Identificarea encefalitelor purulente la ovine sacrificate pentru consum ridică
probleme cu grave implicaţii pentru sănătatea publică prin consumul de produse
obţinute de la astfel de animale. Prezenţa coloniilor bacteriene în structurile
sistemului nervos central atrage atenţia asupra necesităţii completării
investigaţiilor obligatorii pentru acest ţesut cu investigaţii
histologice/bacteriologice efectuate mai des decât până în prezent.
5. Din evaluarea statistică a incidenţei modificărilor/leziunilor în diferitele segmente
de sistem nervos central rezultă importanţa practică a obligativităţii examinării
microscopice a trunchiului cerebral, nu numai pentru evidenţierea
prezenţei/absenţei leziunilor specifice encefalopatiilor spongiforme transmisibile.
6. Ar fi oportună revizuirea circumstanţelor în care examenul histopatologic este
prevăzut ca examen obligatoriu în cadrul Programului acţiunilor de supraveghere,
prevenire, control şi eradicare a bolilor la animale, a celor transmisibile de la
animale la om.
7. Diagnosticarea cazului pozitiv de scrapie la ovina sacrificată pentru consum uman,
în condiţiile unui număr în creştere de probe recoltate de la această categorie
“ ţintă”, este un argument pentru monitorizarea sporită a ovinelor sacrificate în
sistem tradiţional, în condiţiile în care, structura sectorului ovinelor în România
are specificul său.
8. Fără a considera analiza situaţiei infecţiei prionice în judeţul Cluj un studiu
epidemiologic de anvergură, se deduc câteva aspecte utile, în condiţiile în care, la
ora actuală, foaia de parcurs pentru encefalopatii spongiforme transmisibile a
Comisiei Europene stabileşte că unul dintre obiectivele strategice este revizuirea
măsurilor de eradicare în ce ceea priveşte rumegătoarele de talie mică, ţinând cont
de instrumentele de diagnosticare existente şi asigurând nivelul de protecţie al
consumatorului.

Summary
Infectious, traumatic, toxic or degenerative factors from other organs, can induce
pathognomonic lesions in the central nervous system structures (eg, rabies, scrapie) or
cyto-histological changes in the organization of complex nervous tissue and cause
pathological processes characterized in particular by changes in neurons and glial cell
elements at that lesions associated with diagnostic significance in conjunction with
epidemiological data and other laboratory investigations.
The histopathology is aimed at highlighting the key structural changes in the
morphological and histological appearance, in order to identify the mechanisms that
generate local disturbances and their impact on the whole body, correlating clinical
manifestations of sick animals (when they were captured) with lesions detected.
Structural changes, highlighted by macroscopic and microscopic examinations, are
leading to the interpretation of injuries caused by pathogens or aggressors, prioritizing
their contribution to the development of lesions in dynamic or cumulative effect when it
exists.
Nervous forms are frequently reported in sheep, but also very uniform in clinical
manifestation and pathology. For this reason, examination of histopathological changes
of the central nervous system to determine the frequency, preferential localization, the
predominant type of lesion, or the discovery of specific etiological injuries have practical
importance in differential diagnosis.
Transmissible spongiform encephalopathies, along with other major zoonotic
disease localized to the nervous system in sheep, rabies, are diseases for which laboratory
investigations of the central nervous system in sheep are mandatory – specified in the
program of supervision, prevention, control and eradication of animals diseases, those
transmissible from animals to humans, developed the "National Sanitary Veterinary
Authority, Bucharest".
Since 1990 the European Commission adopted a series of measures to protect
human and animal health, the risk of contamination with transmissible spongiform

encephalopathies (TSEs). These measures were based on the safeguard provisions of the
Directives on veterinary control measures.
Given the magnitude of risk posed by certain TSEs, the Commission provides for
increased monitoring programs for goats and sheep, following detection of cases of
bovine spongiform encephalopathy (BSE) in a goat in 2005 and three unusual cases of
TSE in sheep, from which BSE could not be excluded.
Research structure. The thesis comprises 248 pages and is structured in accordance with
current legal provisions; the content includes a number of 121 figures (pictures, graphs,
charts) and 23 tables. In Part I, are the most important and conclusive data in the
literature concerning the histology, physiological and cytopathological regarding the
nervous tissue and aspects of transmissible spongiform encephalopathies in small
ruminants - prion etiology, pathogenesis, clinical and laboratory diagnosis. Part II
includes personal research, laboratory studies aiming to monitor scrapie (Chapter II.1),
histopathological study (Chapter II.2) of the central nervous system, conducted on a fixed
number of brain samples in order identify changes/damages of the nerve tissue, other
than those specific to transmissible spongiform encephalopathies and finally to confirm
two outbreaks of scrapie (Chapter II.3). The bibliography includes 201 titles of scientific
papers from domestic and foreign literature.
Laboratory studies to monitor the scrapie
Chapter II.1. Evaluates the results of laboratory investigations towards scrapie,
comparing each diagnostic method, in terms of sensitivity and specificity, ie detection
and quantification of abnormal PrPres prion protein accumulation in the nervous tissue,
resistant to proteinase K, by immunoassay tests and highlight the presence/absence of
lesions caused by the conversion of PrP in PrPres and its accumulation, by histopathology.
The study looked at lab tests on brain samples from sheep, for supervision and / or
monitoring of transmissible spongiform encephalopathies in small ruminants in Cluj
County, between years 2004-2010.
During the period under study, we examined a total of 1540 sheep, observing the
"target" groups established by legislation on TSEs.

Thus, a total of 610 samples came from dead sheep, 594 samples from emergency
slaughtered sheep (in this category enrolling and sheep slaughtered for diagnostic
confirmation or for the need of elimination from the population, as those with a diagnosis
of infectious epididymitis, Maedi-Visna) and a total of 336 samples came from clinically
healthy sheep slaughtered for human consumption (traditional slaughter for family
consumption).
Following the technical details of the Program for the eradication and monitoring of
transmissible spongiform encephalopathies, for the brain samples collected from dead
and from the emergency slaughter animals (except those unfit for histological processing)
rapid screening was performed and histological examination according to specific
procedure.
Rapid screening method was performed by BIO-RAD TeSeE, which is the
validated method of LSVSA Cluj histopathology laboratory, and accredited by National
Accreditation Body of Romania (RENAR.). It is an immunoassay method which uses
two monoclonal antibodies that allow the qualitative determination of abnormal prion
protein PrPRes, resistant to proteinase K, in the central nervous tissue after distortion and
protein concentration.
Histopathological, for each sample 20 sections were examined from the spinal
bulb, midbrain, cerebellum, cerebral cortex and spinal marrow, processed by paraffin
technique and hematoxylin eosin stained.
Thus, out of 1540 samples undergo brain examination by rapid tests, a total of 541
were subjected to histopathological examination for evidence of lesions characteristic to
scrapie, as follows: 330 samples of brain taken from cadavers (dead sheep on farms,
households, on pasture, during transport, slaughter, or in case of accidents, regardless of
their nature), 208 brain samples taken from emergency slaughtered sheep (in which ante-
mortem clinical examination established the presence of nervous symptoms, or that
cannot move, recumbent, unable to stand or walk without assistance; sheep identified by
the physician, with ante-mortem clinical signs of disease, regardless of disease: infectious
diseases, etc.) and a number of 3 brain samples taken from sheep slaughtered for human
consumption (family).

Of the 1540 tests immuno-enzymatic tests in 1527 showed optical densities below
the threshold (cut-off), thus recording the first negative test results. A total of 13 tests
were initially located just below the optical density threshold and were retested in
duplicate from the original homogenate. For all 13 cases the results were negative for
both duplicates.
The 541 brain samples, processed through paraffin for histopathology, revealed no
specific lesions of transmissible spongiform encephalopathies, in none of the examined
fragments of nervous tissue.
Following these tests (rapid screening, histopathology) has been established that
the brain samples analyzed in 2004-2010 came from sheep free of transmissible
spongiform encephalopathies (classical scrapie, atypical scrapie).
Histopathological study
Chapter II.2 presents the results of a histopathological study of the central nervous
system of a fixed number of brain samples (100) processed histologically for monitoring
scrapie, following the analysis of incidence changes/injuries (other than the specific TSE)
in the central nervous system, the "target" categories of examined sheep, their
classification according to the predominant histological features, development of a
comparative scale assessment, of the distribution and frequency of microscopic lesions in
various parts of the nervous tissue examined, establishing a correlation between
changes/lesions identified and health and/or any distance damage.
The study identified changes / lesions of different types of nervous tissue in 73% of the
examined cases, grouped according to the prevalence of degenerative histopathological
aspects, vascular, non-inflammatory or inflammatory cell reaction.
Briefly, the results of microscopic examination, identified degenerative changes of
the nerve cell body (changes in the nucleus, cytoplasm), vascular disorders (edema,
congestion, hemorrhage, proliferation of endothelial cells, vascular thrombosis), cellular
responses (infiltration with mononuclear infiltration of granulocytes , proliferation of
glial cells, the macrophages, giant cells present), the presence of viral inclusions, images
of cell death (chromatolysis, satellitosis, neuronophagia), metabolic disorders with
accumulation of pigment, presence of parasitic type granulomas.

Making a descending hierarchy of the types of changes/lesions in different
segments of the central nervous system for the 100 reviewed cases, were found the
following:
The most frequent injuries were the vascular type, the result of microscopic
examination is a total of 177, in which images of edema were encountered 84 times, 53
times of congestion, hemorrhage 8 times, in 29 images the vascular endothelial cell
proliferation was captured, and 3 images of thrombosis.
In the hierarchy, are degenerative changes perykarions with a total of 93 images of
"chromatolysis" (45 images that often could be interpreted as nonspecific indicator of
degeneration combined with loss of tinctorial affinity), granular and vacuolar
degeneration (31) and neuronal atrophy with loss of cytoplasmic volume and reduced in
volume (17).
Cellular reactions were found in total of 84, and consisted of mononuclear
infiltration (37), granulocyte infiltration with neutrophils (16), gliocytosis (14),
infiltration with eosinophils (13), and proliferation of macrophages (2) presence of giant
cells (2).
Viral inclusions were captured by two times, intracytoplasmic located.
Were captured: 28 images with cell death with satellitosis (11), neuronophagia (9), and
tigrolysis (8).
Melanic pigment accumulation was found 3 times in the leptomeninges.
Analyzing the distribution of types of changes/lesions on the sheep categories, the
dead and the emergency slaughter represents 97% of the study material, as following:
In both sheep categories, vascular lesions predominated, followed by degenerative
changes of perykarions.
In the case of brain samples from emergency slaughtered sheep, the cell responses
were recorded for all cell types (mononuclear, neutrophil granulocytes, glial cells,
eosinophils). The presence of macrophages and giant cells, parasite-like was found
equally in both categories.
Images of cell death were more common in emergency slaughter sheep, compared
with histological preparations from corpses.

Melanic pigment accumulation was seen in histological preparations from corpses,
but was not seen in preparations obtained from emergency slaughtered sheep.
Parasitic granulomas were found equally in the two categories of sheep.
On histological examination, from sheep slaughtered for human consumption,
were met: chromatolysis type images (2), congestion (2), infiltration with mononuclear
(2), edema (1), satellitosis (1), melanosis (1).
Statistical evaluation of location within the main types of microscopic lesions in
the central nervous system examined segments revealed the following:
Degenerative changes of perykarion, primarily affecting the spinal bulb (22.31%),
then about equally cerebral cortex (19.83%), cerebellum (19%), pons (18.18%) and
quadrigeminal tubercles (16.53%). In a much smaller extent, were affected neurons in the
hippocampus or cervical spinal marrow.
There is an increased incidence of non-inflammatory lesions consisting of edema,
congestion, hemorrhage, especially in the quadrigeminal tubercles (29.37%), bulb
(22.03%), cerebrum (19.21%), cerebral cortex (12.99%) and pons (11.30%), while their
prevalence is much lower in cervical spinal marrow (3.38%) and hippocampus (1.69%).
If inflammatory lesions, spinal bulb was the most affected, the proportion of
34.14%, then quadrigeminal tubercles, in a proportion of 24.39%, cerebellar cortex and
cerebral cortex at a rate of 19.31%, respectively, 12, 50%, much lower percentage of
inflammatory lesions were located: in the hippocampus up to 4.55%, spinal marrow and
pons, each with 3.41% and 2.27%.
Melanic pigment deposition was identified in 100% of cases, with localization in
leptomeninges, in the cerebellum and cerebral hemispheres.
There is no statistically significant difference, in terms of frequency of localized
changes/lesions in the nervous tissue segments examined. Bulbs and quadrigeminal
tubercles are mostly affected, while the hippocampus and spinal marrow are affected in
the slightest degree. Cerebral cortex and cerebellum are changed approximately with the
same frequency. The pons stands the presence of degenerative changes and non-
inflammatory injuries of perykarion and to a much lesser extent inflammatory lesion
occur.

Vascular lesions were predominant in 41% of cases, followed in the hierarchy by
inflammatory lesions, with a rate of 16%. These include inflammation of parasitic nature
(8%), bacterial inflammation (6%) and inflammation of viral nature (2%). For 11% of
cases, changes were identified whose etiology could not be determined and were
attributed to diseases and pathological conditions in the visceral organs, such as
metabolic, infectious and/or parasitic. Accumulation of melanic pigment was observed in
3% of cases and changes in cerebrospinal fluid in 2% of cases. No changes were
observed in nerve tissue in the central nervous system segments examined in 27% of
cases.
Non-inflammatory edema was diagnosed as the predominant change in 37 cases,
and consisted of images showing the enlarging of Wircoff-Robin space, sometimes with
the presence of poorly colored liquid with vacuolar appearance, capillaries or arterioles
having a reduced lumen due to transudation. Base edema, is the vascular endothelial
injury with a severity that increase vascular permeability. The fluid will disperse between
cells in response to hydrostatic pressure from the movement of the brain and from other
tissues.
They found differences in susceptibility to edema, between gray and white matter
and the vulnerability of different areas was also different and consisted of images of
intracellular edema, hypertrophy translated into astrocytes, oligodendrocytes
accompanied sometimes by the reaction of the white matter (nucleus increased, less
dense, hypochromic) vasogenic edema (intercellular) without major changes in the gray
matter, but white matter, which provides a low resistance to passage of edema fluid, is
textured with vacuolar appearance.
Frequently, there was swelling in the ependymal channel epithelium, epithelial
cells are invaded by liquid, cell nucleus pushed to the apical pole, sometimes pyknotic,
other times the epithelial cells were detached, and the substance surrounding nerve had
spongy appearance.
Cerebellum, particularly in the area of Purkinje cells, was the seat of the displace
and compress of the cells. Often, the swelling was accompanied by a discreet gliocytosis
and/or lymphocytosis.

Hemorrhage, has prevailed in four cases, the issues encountered by diapedesis,
bleeding without specificity in the topographic distribution with perivascular remaining
erythrocytes, with or without hypertrophy and endothelial cell nucleus, and bleeding per
rexis with bilateral and symmetric location in the cortex brain. For two of these cases, the
bacteriological examination was positive for Clostridium spp, and another case, showed
this massive infestation of Oestrus ovis larvae that were in the circulation that reached
skull. Images of edema and hemorrhage were not accompanied by inflammatory changes.
Attention was paid to central nervous system inflammatory lesions, very important
for veterinary pathology since most of them are fatal and that histological examination
should guide or determine etiologic diagnosis.
Parasitic inflammation represented half (50%) of the inflammatory identified
lesions. Bacterial inflammations were diagnosed in 25% because of Listeria and other
purulent inflammation 12.5%. Viral inflammations have been identified in a proportion
of 12.5% due to rabies.
Histopathological diagnosis of parasitic encephalitis was correlated, in the case of
6 emergency slaughtered sheep, with nervous signaling disorders, with spins in the riding
and balance disorders as a result of compression exerted in the cerebral hemispheres,
respectively cerebellum or spinal bulb.
The picture was pathologically correlated with the stage of disease evolution,
showing a pronounced polymorphism. We identified the presence of neutrophilic and
eosinophilic infiltrate, in addition, in two of the cases, parasitic granuloma with giant
cell, vascular-conjunctiva and macrophage reaction with the cytoplasm 'sparkling', the
remains of parasites, three of the cases were diagnosed as parasite migration encephalitis,
with irregular foci of necrosis, eosinophilic infiltration, the two encephalitis cases were
accompanied by leptomeningitis eosinophilia. Damage caused by parasites was found in
the bulb, quadrigeminal tubercles, cerebral cortex, deck, hippocampus and cervical spinal
bone.
For two dead sheep, veterinary documents did not specify the nervous symptoms,
brain samples were processed only for monitoring scrapie, although histopathological
study revealed advanced lesions, resulting in giant cell development, multinucleated,
with the cytoplasm focused on detritus and loaded with vacuoles and tissue debris, and

the presence of parasitic thrombus.
Bacterial encephalitis, accounted for 37.5% of inflammatory lesions, of which
25% were identified as listeria-like.
At a sheep without clinical signs, slaughtered for human consumption,
inflammation was predominantly with monocytes, which indicates an acute stage of
infection which, logically, should be accompanied by nervous symptoms.
At the emergency slaughter sheep, because they had clinically abnormal head
position, the lesions were located in the spinal bulb and cerebellum, and consisted of foci
of liquefaction necrosis with neutrophilic infiltration with monocytes and lympho-
histiocytic perivascular sleeves.
A percentage of 12.5% of bacterial inflammation were observed in emergency
slaughtered sheep (2 cases), and were purulent encephalitis with massive infiltration of
granulocytes neutrophils, leptomeningitis, and the presence of bacteria arranged in nests,
in a case with localization in the spinal bulb, and in the second case in the cerebellum, in
both cases bacteriological examination was positive for β-hemolytic strains of E. coli.
Infiltrating stage, with purulent exudate character were correlated with disease duration,
both clinically diagnosed cases manifested by nervous symptoms and prolonged
decubitus.
The diagnosis of viral encephalitis, respectively, rabies was established in two
cases showed nervous symptoms, which were slaughtered by emergency. In both cases,
in addition to lympho-histiocytic encephalitis, Babes Negri corpuscles were identified.
Alterative lesions and cell proliferation were moderate compared with clinical
manifestations.
Nonspecific encephalitis has been described in five sheep, emergency slaughtered,
and 6 dead sheep. Intensity was less relevant and considered to be associated with disease
states with locations in other organs. Degenerative changes were identified perykarions
(eccentric nuclei, cromatolisis, outlining Nissl granules, pyknosis, neuronal lysis, with
changes in astrocytes and capillary endothelium in the vicinity of affected neurons) of the
spinal bulb, quadrigeminal tubercle, cerebral cortex and cerebellum, glial proliferation,
neuronophagia with microglial phagocytes gathered around the body fragments,

degenerate neural perivascular edema and / or nervous substance, the diapedesis
bleeding, presence of eosinophilic infiltrate in the lumen of capillaries and arterioles.
Melanosis was found in two bodies, and a sheep slaughtered for human
consumption. Pigment deposition was shown in a manner analogous to other lysosomal
storage diseases; a well-defined limiting membrane is not obvious and was identified in
cerebellum and leptomeninges of the hemispheres. In one case perivascular melanocytes
were present but in no cases melanocytes were identified in the nervous substance.
In two bodies, we found an increase in cerebrospinal fluid, translated by dilating
aqueduct, sometimes associated with edema surrounding brain substance.
In summary, the most affected by changes/lesions in nervous tissue were dead
sheep, i.e. 40 cases, and emergency slaughter sheep, 31 cases, and of sheep slaughtered
for human consumption, although two cases were identified with lesions, should be noted
the very small number of histopathological examinations were performed on sheep in this
category namely, 3. Moreover, three cases were processed to obtain histological
preparations of nervous tissue for microscopic examination of reference, which are
received in the pathology laboratory with accompanying documents, certifying the state
of clinical healthy animals slaughtered for human consumption, and heads were been
submitted for rapid screening by immuno-enzyme test.
Changes/injuries were diagnosed in the nervous tissue from emergency
slaughtered sheep, which do not have nervous symptoms or macroscopic changes in
central nervous system, and clinically healthy sheep slaughtered for human consumption.
Histopathological study revealed a higher incidence of changes / central nervous
system lesions, compared with clinical case studies, which are commonly found in sheep,
often asymptomatic or evolving clinical signs are less obvious, leading to their non-
reporting by the owner or by veterinarians.
Presence of changes / lesions in the central nervous system, associated with
primary lesions in other tissues and organs, indicate that blood-brain barrier exists, but
not for any of the pathogenic organism.

Confirmation of two outbreaks of scrapie
Of the 158 brain samples examined in 2011 by immune-enzyme test BIO-RAD, a
rate of 39.24%, i.e. a total of 62 samples were histologically processed for evidence of
specific lesions of transmissible spongiform encephalopathies (classical scrapie, atypical
scrapie).
The first positive diagnosis of scrapie cases, the Pathology Laboratory,
specializing in the diagnosis of transmissible spongiform encephalopathies in LSVSA
Cluj, in 2011, was the motivation for further research on forms of transmissible
spongiform encephalopathies in small ruminants.
In Chapter II.3 we presents the results of the diagnostic protocol adopted in the
two positive cases, representing the two "target" categories, comparing the performance
results of laboratory tests performed on these positive cases, and epidemiological aspects
of scrapie in the Cluj county, in the context of evolution of the disease in Romania.
'Target' categories, represented by the two positive cases were: clinically healthy
sheep, slaughtered for human consumption, and emergency slaughter sheep following
clinical examination that was showing: nervous symptoms that are seen with ataxia, head
support on various objects, generalized tremors, stiff neck, increased self-mutilated
pruritus in the upper limbs, ataxia, progressive weakness, lying on the ground.
As a result of framing in the two "target" categories, according to the compulsory
work protocol - we proceeded to the taking of samples that were targeted for laboratory
investigations.
From sheep with nervous symptoms was taken the whole brain, and the working
protocol consisted of nervous tissue sampling, from the obex region, for rapid screening
of scrapie by immuno-enzyme test, pons fragments, cerebral cortex, cerebellum for
scrapie histopathology, brainstem fragments (loose tissue, preserved by freezing) for
Western Blot confirmatory test, and pons fragments for scrapie immunohistochemistry.
From clinically healthy sheep, we collected brainstem by foramen magnum
technique, and have taken: nervous tissue sample for scrapie immunoassay test from obex
region and brain stem fragments for Western blot confirmatory test.

Western Blot confirmatory test was done by digesting the cellular prion protein
(PrPc), followed by purification and concentration of PrPres specific for this disease. PrPres
detection was done by electrophoretic migration followed by immunoblotting.
Immunohistochemistry test for scrapie was performed on histological sections
using a highly specific monoclonal antibody for PrPres - monoclonal primary antibody on
anti PrPSC mouse.
Immunoassay tests - had resulted in initial testing, for both samples, optical
density values above the threshold (cut-off), resulting in large reports in favor of optical
density recorded for those samples compared to cut-off (1.768/0.218 OD/C.OFF for the
case with nervous signs, 1.692/0.220 for the case without clinical signs).
To check the accuracy of the results from the initial homogenate, both samples
were retested by immuno-enzymatic test in duplicate. In the case with nervous
symptoms, the optical density was well above the threshold, resulting in reports over
optical densities recorded for the sample, compared with cut-off (3.601, 3.634/0.219).
Optical densities were obtained over the threshold (cut-off) even if no clinical symptoms
(1.670, 2.306/0.220). The results were interpreted as positive for scrapie.
High value of optical densities obtained in nervous tissue samples taken from the
case with nervous symptoms, is due to more increased quantity of PrPres detected in
nervous tissue compared to the amount of PrPres if present in nervous tissue without
clinical signs.
Obtain optical density above the threshold in the clinically healthy sheep
slaughtered for human consumption, shows sensitivity and specificity of immuno-
enzymatic test for rapid screening in detecting PrPres abnormal prion protein in infected
nervous tissue.
For both samples, immunoblotting yielded specific profiles, with the presence of
large patches only on molecular weight protein band of 27-30 kDa, present in the sample
treated with proteinase K. Electrophoretic migration resulting profiles, followed by
immunoblotting, demonstrating digestion of PrPc, and disease-specific prion protein
transformation into a fragment resistant to proteinase, with low molecular weight by
removing amino-terminal end of the protein, the two upper bands correspond to mono-

and di-glycosylated forms (27-30 kDa) and lower band corresponds to non-glycosylated
form.
Histopathological examination identified the presence of specific vacuolation,
located in neurons perykarion, and in the gray matter neuropile with symmetric
distribution, bilateral. Perykarions look relaxed, with one or more vacuoles, round or
oval, with net and regularly shape, that merging, resulting in spongiform appearance of
gray matter. Images are found with neuronal necrosis, neurons with pyknotic nuclei,
homogeneous cytoplasm, intense eosinophilia, discrete glial proliferation and
perivascular amyloid deposits, no inflammatory reactions were observed. Dorsal nucleus
of the vague proved to be the place of choice for specific vacuolation.
Highlighting lesions demonstrate that prions penetration in the central nervous
system is followed by conversion of normal PrPc protein produced by neurons and glial
cells in PrPsc, and its accumulation, followed by characteristic histopathological changes
responsible for clinical disorders.
Immunohistochemistry performed on the case with nervous symptoms, in
histological sections made from fragments of brain stem, were identified the storage
points, PrPres granules arbitrarily arranged in several foci or diffuse in neuropile and
perykarions, with different intensity and distribution of immuno-reactivity.
The analysis results show the sensitivity and specificity of the immuno-enzymatic
test for rapid screening in the detection of abnormal prion protein PrPres in infected
nervous tissue by detecting the disease in sheep clinically healthy slaughtered for human
consumption, and that there is correlation between the amount of abnormal prion protein
PrPres accumulated in nervous tissue, and the onset of clinical signs, as evidenced by the
high value of optical densities obtained in samples of nervous tissue, taken from the case
with nervous symptoms, compared with those obtained in the case without clinical
evidence.
The study revealed that scrapie can evolve in the absence of clinical signs, which
means that occult prion infection in animals are much more widespread than known
clinical cases. Given that the positive diagnosis of the sheep slaughtered for human
consumption was made through routine monitoring, emphasizes its importance.

General conclusions
1. Histopathological study, resulted in detection of changes / lesions of the central
nervous system in 73% of the cases examined, highlights the importance of
examining the tissue with greater attention, both macroscopically and by sampling
for histopathological study to identify primary lesions and those associated.
2. Changes / lesions in nervous tissue were diagnosed in emergency slaughter sheep,
which showed no nervous symptoms or macroscopic changes in central nervous
system, and in clinically healthy sheep slaughtered for human consumption.
3. Factors as: infectious, traumatic, toxic or degenerative, in other organs induce
cyto-histological changes in the complex organization of nervous tissue, with the
emergence of pathological processes, showing that blood-brain barrier exists, but
not for any of the pathogenic organism.
4. Identification of purulent encephalitis in sheep slaughtered for consumption raises
serious public health implications by eating products from such animals. The
presence of bacterial colonies in the central nervous system structures, points out
the need for mandatory investigations of this tissue, with
histological/bacteriological investigations performed more frequently than at
present.
5. For statistical evaluation of the changes/lesions incidence, in different segments of
the central nervous system is resulting the practical importance of the obligation to
perform the microscopic examination of brain stem, not only for the
presence/absence of specific lesions of transmissible spongiform
encephalopathies.
6. It would be appropriate to review the circumstances in which histopathology is
said mandatory - in the "Program of the surveillance, prevention, control and
eradication of animal diseases, those transmitted from animals to humans."
7. Positive case of scrapie diagnosis in sheep slaughtered for human consumption, in
an increasing number of samples collected from this "target" category is an
argument for increased monitoring of sheep slaughtered in the traditional system,
while the structure of the sheep sector in Romania has its specifics.

8. Without considering the situation of prion infection in Cluj County as a large
epidemiological study, several useful aspects to be deducted, provided that, at
present, the roadmap for transmissible spongiform encephalopathies of the
European Commission states that one of the strategic objectives is to review the
eradication measures for small ruminants which by taking into account the
existing diagnostic tools and ensuring the protection of the consumers.
